The Service Connect Order form is an agreement between the customer and RMU for services rendered and electric energy and/or water consumed in accordance with the schedule of rates published for the class of service which is supplied and further agrees to be bound by the rules and regulations governing the service.

- Applicant(s) drivers license or a valid Photo ID is required.
- Mail completed form to 102 W. 9th Street, P.O. Box 767, Rolla, MO 65402-0767, drop off at RMU Business Office or submit below. Make sure a copy of applicant(s) drivers license/Photo ID accompanies the completed form.
- Electronic Signature Agreement – The service connection forms below are in writable PDF form. If you choose to fill the form out electronically, you agree your electronic signature is the legal equivalent of your manual signature in accordance with RSMo; Section 432.230.1. By electronically signing, you consent to be legally bound by the content of this document.
- Once submitted we will process your application during regular business hours. If applications are submitted after 2 PM they will be processed on the next business day.
