Customer Service Application Forms

The Service Connect Order form is an agreement between the customer and RMU for services rendered and electric energy and/or water consumed in accordance with the schedule of rates published for the class of service which is supplied and further agrees to be bound by the rules and regulations governing the service.

  • New customer service deposit may be required. Please check “Rates and Fees” link for deposit requirements.
  • Applicant(s) drivers license or a valid Photo ID is required.
  • Once submitted we will process your application during regular business hours. If applications are submitted after 12 PM they will be processed on the next business day.
